A lack of safe, reliable & affordable transport has been consistently identified as a major barrier to older people’s wellbeing in Stoke. One of the impacts is that without such transport, people miss important health and wellbeing appointments because they are unable to get there. This has serious consequences for their health and wellbeing, and increases pressure on the NHS, as well as families.
Carelink Plus is a new project that will provide a vital, currently lacking, transport link for our clients to attend such appointments where otherwise they would be unable to attend. Carelink staff will provide transport/chaperone duties 3 days a week on a flexible basis. Where clients identify upcoming appointments which they are struggling to get transport for, we will provide the level of support that they need to overcome this barrier, thereby providing a bespoke, needs-led service. This may range from just transport, to transport plus chaperone support to the appointment where notes can be taken and shared with families if needed. The funding will pay for the salary costs to do this, plus transport costs (fuel, insurance, parking).
